Where to get the new 5527/1 form

April 22, 1998 -- Effective June 1, 1998, the old 3-page ICR is being replaced by a new 4-page report (which includes six addenda pages).

If you would like to see a full version of these forms, click here. This is a PDF file which you can view and print using Adobe Acrobat which you can download for free. If you want to use these PDF files to fill out your Incident Reports, you'll need to buy a separate program from Adobe.

A simpler solution is to just download STARS for Windows. STARS for Windows will produce all 10 pages of the new Incident Report, in strict compliance with the new reporting requirements. Shown below are miniature versions of these forms.
